Check for the five to six rights; right name, right drug, right dose, right time for administration, right route of administration, right documentation. Check for drug allergy. Determine amount of diluent to be added to achieve correct dose by reading label instructions. Clean rubber top of diluent vile and medication vile with alcohol swab. Draw air into syringe equal to amount of diluent to be withdrawn. Inject air into diluent vile and withdraw diluent. Inject diluent into medication vile. Invert medication vile several times to dissolve medication. Draw air into syringe equal to amount of medication to be withdrawn. Do this my inverting vile and hold at eye level while withdrawing the correct dosage of the drug into the syringe. Inject air into medication vile and withdraw medication. Withdraw the needle from the vile and replace the cap over the needle to maintain sterility. Do not use passive scoop technique with a sterile needle. Check label on the medication three times when taking container from shelf just before withdrawing med and before returning container or before discarding. Properly dispose of medication vile and wash hands. Ampules Check date on medication order and verify it's accuracy by checking it against the doctor's order. Check for the six rights; right name, right drug, right dose, right time, right route and right documentation. Check patient's record for any drug allergies. Gather all equipment. Check label on medication three times before administering it. When taking the container from the shelf just before pouring and before returning container or discarding wrapper. Flick upper stem of ampule several times with finger nail. Make certain all medication is out of the ampule stem. Place an alcohol swab four by four or ampule breaking device on the neck and break off the top by bending the top towards you with a quick snap. This allows the glass to break away from you. Assemble syringe and needle. Remove cap from needle, insert needle into the ampule, invert and withdraw amount of drug required. Do not inject air into the ampule. Replace cap over needle. Dispose of ampule in sharps container; wash hands. >> Administration of oral medications. Wash hands. Gather equipment. Check label on medications three times before administering when taking container from shelf, just before pouring, before returning container or discarding wrapper. Prepare the correct amount of medication without touching the container and contaminating the medication. If administering tablets from a multi dose bottle, pour the required number into the bottle cap and transfer the medication into a medication cup. If administering unit dose tablets, keep the tablet in the package and place in directly into the medication cup. If administering a liquid medication, remove the cap, and place it upside down to prevent contamination. Hold the cup at eye level and fill to desired level using the meniscus as a guide. Replace cap. If administering a unit dose liquid, carry the liquid medication to the patient's room where you will administer the med. Wipe. Confirm at least two forms of patient identification on the wristband and compare to bar code scanner. Assist the patient to a sitting position. Give the patient his medication and appropriate liquid as needed. Stay with the patient until he has swallowed all of the medication. Wash hands. And document
